Qualifications
If you experience a chronic illness or disability, or if you suffer from memory loss or confusion (like Alzheimer’s) and cannot perform at least 3 of these 6 activities without help—
- eating
- personal grooming and hygiene
- dressing and undressing
- toilet activities
- moving from one place to another
- elimination
—you are eligible for LTC services.
Applying is easy
Call us at 800.279.4000 for a claim form. It requires information from you and your treating physician and must be filed within 90 days of becoming functionally incapacitated. Then simply…
- Fill out your portion of the claim form.
- Ask your physician to complete his or her portion.
- Make sure you satisfy your elimination period, beginning after your physician completes the required claim form (remember, you pay for LTC services during this time).
The Trust pays 75% of the covered charges
We pay up to 75% of covered charges incurred up to the maximum daily benefit shown on your benefit summary. You are responsible for 25% of the charges, plus any charges exceeding the maximum benefit.
The Trust does not pay for LTC benefits that would be covered by sources such as Medicare, group health insurance, or worker’s compensation, whether or not you apply for them. If your expenses are only partially covered through other sources, we will calculate the amount we would have paid if you had no other coverage. Then we will pay the remaining portion of your covered charges up to our calculated maximum.
You will receive benefits while you are functionally incapacitated until your lifetime maximum benefit (total amount of dollars available for your LTC services over your lifetime) is exhausted.
