If you've been diagnosed with a serious medical condition, you're probably more than a little anxious and confused. Where should you turn? How do you get the information you need to get the most appropriate care for your condition?
By enrolling in a Trust Care Management program, you'll get compassionate support in navigating the often-complex health care system. We'll get you up to speed on the standards of care for your specific diagnosis so you can make informed choices.
If you like, you can receive one-on-one guidance right over the phone from an experienced Trust care manager, who will serve as your advocate to help you better manage your care. So you'll never have to feel alone.
Your care manager can:
- Explain how your current health condition may affect other health risk factors.
- Provide educational materials that emphasize appropriate treatment and what you need to do to stay in compliance.
- Focus on the necessary changes in your lifestyle, such as improved physical activity, better food choices, and reduced stress.
- Talk over the types of potential side effects that may occur as a result of your treatment, and how to effectively manage them.
- Give you suggestions on how to speak with your provider about nationally recommended standards of care.
- Answer your questions or concerns about medications.
- Complement your providers' care.

Am I eligible to participate?
The answer is yes if you are 18 or older (Pediatric diabetes program is for kids under 17), with a diagnosed health condition. You must have a WEA Trust Plan as your primary insurer.
