By Mail

To change your beneficiary or beneficiaries on your life insurance, download the Life Insurance Beneficiary Form. Complete and mail to: WEA Trust, P.O. Box 7338, Madison, WI 53707-7338. You will receive confirmation of your new designation in writing. Beneficiary changes are in effect the date you sign the form.

Online

Log in to Change Your Beneficiaries. You will need your social security number**, and employer ID and password. The change is in effect the date you submit the online form.

** Please use your Subscriber Number instead of your social security number. Thank you.

File a claim

A claimant must send written notice of the claim within the following time periods:

• Death – written notice must be given within 90 days after the date of death.
• Accidental dismemberment – written notice must be given within 90 days after the covered employee's date of loss.
• Waiver of premium – written notice must be given after 9 months but within 12 months after the last date the employee was actively at work for the employer, performing all job duties.

Payment

A lump sum is paid to the beneficiary or beneficiaries on record with us within 30 days after we receive all required documents needed for the claim.
