That's why the Trust PPP for state health plan members packages an attractive dental plan alongside your health plan. We take care of your smile by offering the following dental benefits:
- 100% coverage on preventive services such as exams, cleanings, fluoride treatments, bite-wing X-rays, and sealants.
- 80% coverage on restorative services such as fillings, extractions, and periodontal treatment.
- 50% coverage for crowns and onlays (excludes implant crowns).
- Orthodontia paid at 50% up to a lifetime of $1,500.
- A $1,000 annual individual maximum with no deductible.
Coverage For Any Dentist
With the Trust PPP dental plan, you can see any dentist and still receive benefits. There is no deductible for dental services, even if your dentist is out-of-network. Receiving care from one of our Network providers will ensure the lowest out-of-pocket expense as benefits for seeing a non-network dentist are subject to our reasonable and customary fee schedule.
