| Answer: | If you ever have a question after receiving an Explanation of Benefits form, you can call our Dental Customer Service Department at (800) 279-4000. When you call, we will need your name, subscriber number (usually your Social Security number), patient name, date of birth, and the date of service in question. You may find it to your advantage to have the Explanation of Benefits in front of you when you call us. |
