How do I know if my child is still eligible for insurance coverage through my plan?

Answer:

If you are covered by the dental policy, your dependents under the age of 19 are also covered. Some school groups extend coverage to dependents beyond age 19. Your Benefit Summary indicates to what age dependents are covered. When your dependent nears the limiting age, we will send you a questionnaire requesting updated information about that dependent. You can either complete and return the questionnaire or call one of our eligibility representatives and provide the information over the phone.
