Question:
Why do you need medical records?
Answer:
Your disability policy requires that you provide contemporaneous objective medical and/or psychiatric information that documents that you are unable to perform the material and substantial duties of your own occupation. A physician's declaration that you are disabled, without accompanying objective medical and/or psychiatric evidence, is not sufficient to prove that you are disabled. Therefore, we need your medical records to adjudicate your claim.
