Question:	What information do I need to submit before a payment is issued?
Answer:	We require ongoing documentation of your disability and compliance with all policy provisions in order to issue benefits. Generally, we require monthly supplemental forms to be completed by you and your treating physician before we issue a disability benefit. We will include these supplemental forms with your benefit statement.
