| Question: | How do I change my designated beneficiary? |
| Answer: | To change your beneficiary, you can simply send a written note to us indicating who you want as your beneficiary. Make sure that your note also includes the date, your name and Social Security number, your school district name, and your signature.
You may also download a Life Insurance Beneficiary Form to change your beneficiary. After downloading the form, fill it out and mail it to: WEA Trust, P.O. Box 7338, Madison, WI 53707-7338. When we receive your beneficiary change request, we will confirm your new designation in writing.
These types of written changes will be effective on the earlier of these dates:
- The date you signed the written notice.
- A date some time in the future if your written notice specifies such a future date.
As an alternative, you can also change your beneficiary online by clicking here. When you change your beneficiary online, the change is effective on the date you submit the online form. |
