Why it's important to call us first
We do not require that you obtain our preauthorization before receiving any dental services. However, if you want to know in advance whether a dental service will be covered and how much we will reimburse, we encourage you to ask your dentist to submit your treatment plan to us in writing.
When submitting this information, your dentist must include the procedure code and fee for each planned dental service. We will review the proposed treatment plan and let you and your dentist know, in writing, which services are covered and how much will be reimbursed.
