Diabetes care (Adults)

Learn how to live a healthy life and keep your diabetes in check by enrolling in our adult diabetes program. When you participate, you will work one-on-one with a registered nurse from the Trust who can:
- Help you understand how diabetes can affect other conditions like high blood pressure, high cholesterol, kidney disease, and heart disease.
- Give you suggestions on how to talk to your doctor about your care.
- Work with your health care provider to ensure your care is consistent with nationally recommended treatment standards.
- Talk to you about how to monitor your diabetes to avoid complications.
- Explain how eating and exercise affect diabetes.
- Answer your questions about medications.
- Provide you with record-keeping tools for tracking your condition.
Can I participate?
You must be 18 to 64 years old with diabetes and have a WEA Trust health plan as your primary insurer.
