Heart care

Learn how to live a healthy life and keep your heart disease in check by enrolling in our adult heart care program. When you participate, you will work one-on-one with a care manager from the Trust who can:
- Help you understand how conditions like high blood pressure, high cholesterol, and diabetes affect heart disease.
- Give you suggestions on how to talk to your doctor about your care.
- Work with your health care provider to ensure your care is consistent with nationally recommended treatment standards.
- Talk to you about how to monitor your heart disease to help prevent complications.
- Explain how eating and exercise affect your heart disease.
- Answer your questions about medications.

Can I participate?
To be eligible for this program, you must be 18 to 64 years old with heart disease and have a WEA Trust health plan as your primary insurer.
