If you would like to change your beneficiary or beneficiaries on your life insurance, you may do so by downloading this form, completing it, and mailing it to: WEA Trust, P.O. Box 7338, Madison, WI 53707-7338. When we receive your beneficiary change request, we will confirm your new designation in writing.
Your beneficiary change will be effective as of the date you signed the beneficiary form.
As an alternative, you can also change your beneficiary online by clicking here. When you change your beneficiary online, the change is effective on the date you submit the online form. |
