Filing a claim

To initiate a claim for long term disability benefits, you must send us written notice of your claim as soon as possible, but no later than 90 days after becoming disabled. This notice alerts us to the existence and nature of your claim. It also allows us to obtain the information and documents we need to process your claim in a timely manner.

You may download a claim form from our Web site by clicking here. You may also contact your employer or call our Disability Department at (800) 279-4000.

Note: We require medical records to evaluate your claim for benefits. Some providers will supply the requested information to us free of charge as a courtesy to their patient. Regrettably, other providers charge fees for the reproduction and transmission of medical record information. According to the express terms of the long term disability policy, we do not reimburse for the cost of medical records. Any charges for the release of this information are your responsibility.
