Filing a claim
You can get the necessary claim forms for filing a claim for long term care benefits by calling us. The forms require information from both you and your treating physician. You should file your claim as soon as possible, but you must file within 90 days of becoming functionally incapacitated. If you are unable to file a claim yourself, you can have someone file it on your behalf.
