Qualifying for benefits

Generally, you must have a severe cognitive impairment, such as Alzheimer’s disease, or the inability to perform, without substantial assistance from another person, at least three of six activities of daily living (ADLs) that are defined by your long term care policy. These ADLs include:
  • Bathing
  • Continence
  • Dressing
  • Eating
  • Toileting
  • Transferring
Your physician must complete a form certifying that you are unable to care for yourself due to your functional incapacity and that you’re incapable of performing three of the previously mentioned ADLs. Benefits will begin after you have satisfied a 30-day elimination period.

Your 30-day elimination period cannot begin until your physician completes the required certification form. It is in your best interest that the certification form be completed as soon as possible. Please keep in mind that benefits are not paid for services you receive during the elimination period.
