Advance planning: Spelling out what you want


You’re strongly encouraged to have documents prepared that carefully set out what your wishes are in case of terminal illness and/or if you are unable to make decisions for yourself.
These documents are called advance directives. They consist of a living will and a durable power of attorney.
Living wills are written instructions that explain a person's wishes regarding health care in the case of a terminal illness and should be as specific as possible.
A durable power of attorney names a person to make decisions for you if you become unable to do so.
