Home Health Plan How Your Plan Works
Using your Preferred Provider Plan (Trust Preferred)

Trust Preferred is an easy-to-use, comprehensive health plan with a broad selection of health care providers.

It gives you and your family members the freedom to use the physician, clinic, or hospital of your choice. You do not need to designate a primary care provider; however, we encourage you to develop an ongoing partnership with a primary care provider so he or she can coordinate the care you receive.

Single statewide Network
Trust Preferred offers a single statewide Network made up of a broad array of physicians, clinics, and hospitals. When you use these Network providers, you will receive the maximum reimbursement amount available.

If you choose to use a non-network provider, you'll pay a higher out-of-pocket cost. Contact our Customer Service Department at (800) 279-4000 or select Find a Doctor to verify that your provider is part of the Trust Preferred Network.

Note that coverage for some services requires preauthorization by the Trust.

If you travel
In addition, we have contracted with many providers outside Wisconsin to make it convenient for our members who live and travel outside the state to obtain covered services from Network providers. When you receive covered services from these Network providers, your coinsurance, deductible, and copayment amounts will be the same as those for Network providers within the state.

Urgent care
All urgent care services are reimbursed at the same coinsurance and deductible amounts that apply to Network providers and are subject to the urgent care copayment, even if the care is received from a non-network provider. However, in order to guarantee the maximum reimbursement for your services, you should access care through a Network urgent care facility, as services received from a non-network provider will be subject to our reasonable and customary fee limits and all applicable policy provisions.

Emergency room
All emergency medical services are reimbursed at the same coinsurance, deductible, and copayment amounts as if received from a Network provider, subject to our reasonable and customary fee limits and all applicable policy provisions.
