Eligible members can earn $150!
As part of our Trust Rewards program, eligible State Health Plan members can earn a $150 Visa® prepaid card*. All you have to do is take our online health assessment AND submit verifiable results for your blood pressure, body mass index, cholesterol, and glucose values. We will also take biometric results submitted through the state's work-site collection vendor. See wellwisconsin.wi.gov for a schedule of work-site collection events. If you completed your biometrics through the state vendor, click here to read what’s next.
Our Trust In Your Health wellness program offers much more than a health assessment.
