State Health Plan Members - Trust Rewards

Eligible members can earn $150!

As part of our Trust Rewards program, eligible State Health Plan members can earn a $150 Visa® prepaid card*. All you have to do is take our online health assessment AND submit verifiable results for your blood pressure, body mass index, cholesterol, and glucose values. What are verifiable results?

If you are unable to attend one of the state's work-site screenings, you may download this form to record your biometric results. Please note, your doctor must enter the information, sign the form, and return it to us for the results to be valid.

Am I eligible to join?

If you're a WEA Trust health plan subscriber, a spouse or domestic partner who has the Trust health insurance, you can use any and all of these services.
