After you are discharged from the hospital, it can be stressful and difficult to return to your daily life and activities. In order to remain healthy, its important to fully understand your discharge care, including medication instructions and knowing who or when to reach out if you need help. According to the Centers for Disease Control and Prevention, nearly 20% of patients experience adverse event or side effect within three weeks of being discharged – and over 75% of those events could have been prevented. 

Our post-hospitalization care management program helps you get better faster by monitoring your progress after you are discharged from the hospital and providing you the support and resources when you need it most. Our clinically proven program combines the ease of automated mobile messages with a team of accessible and supportive WEA Trust Care Management staff to track and support your journey to improved health.

The WEA Trust Care Management Team is available throughout your journey for support. They are your dedicated point of contact to help with your care. Your dedicated Care Management Team will reach out if you show worsening or emergent signs or symptoms. The team is your support system that is able to assist in managing your condition, helping you find a provider, preparing for medical visits and more.

Program Details

How often will I receive communication?

You will receive communication from the Care Management Team based on your preferred method and time of delivery. Members can receive messages twice a week or weekly. Per your needs and progress, communication frequency can be adjusted.

Below are some sample messages:

  • What is your mood now? Please respond on a scale from 0 (very sad) to 10 (very happy).

  • How often have you felt depressed, down, or hopeless? Reply 0: if not at all, 1: sometimes, 2: often, 3: nearly every day.

How can I change my communication preference?

You can change your communication preference (like frequency, timing, or calls versus texts) by contacting the Care Management Team. Should you no longer wish to receive messages, you can text "Stop" at any time on your mobile phone or press " * " (star) on the keypad on your landline phone.


What level of engagement is required?

Engagement is simple and convenient. You can opt to receive questions from a landline or mobile phone, meaning you may receive an automated call or text message of the question. You can also schedule when you’d like to receive the communication. Engagement is important in driving successful outcomes.


How does Post-Hospitalization Discharge Care Management monitor my condition?

Post-Hospitalization, or after you are discharged from the hospital, Remote Health Monitoring uses your responses to the automated messages to identify your risk of readmission and trigger outreach should you have:

  • Worsening symptoms
  • A request to speak with a nurse about medications
  • Questions regarding appointments
  • Questions about additional services

The intervention monitors long-term health to provide you with an objective way of tracking your progress and allows your Care Manager to offer resources based on the type of care appropriate for your level of need. 

Your self-reported updates help assist the WEA Trust Care Management Team in providing additional education on your condition, your treatment options, additional tools and resources and more.
