To help answer some common questions providers may have regarding WEA Trust's exit from the health insurance market, please review our

Maddy Provider Portal is Disabled

As of July 31, 2022, the portal is not longer available. The Provider interactive voice response system (IVR) will be available until March 31, 2023.

We have set up this transition page for access to the most critical information providers need.

Authorization Request Forms

Please submit authorization requests using the links and information below to download, print and fax.


Submit all Corrected Claims Electronically

  • Void and replacement claims should be submitted via Electronic Claims Submission (EDI) 
    Electronic Claims Payor ID#: 39151

  • If you are a paper-only submitter, please continue to send to:

  • For any other claims correspondence use the Claim Resubmission Form.

Provider Explanation of Payment (EOP) Request Form
 

If you would like duplicate copies of an Explanation of Payment (EOP), please complete this EOP Request Form and submit it via email to CustomerServiceCorrespondence@WEAtrust.com

  • Please submit the email with the subject line: Provider EOP Request.

NOTE: If you receive an electronic EOP, we are unable to provide specific details. You will need to contact your IT department for that information.

After December 1, 2022, our Customer Service Team will no longer answer phone calls from providers. The Provider IVR is your main source to obtain member eligibility, benefits, claim status and authorization status for all of your inquires.

We suggest selecting the faxback option for the fastest and best results on all inquiries. Please call the Provider IVR at 800.279.4090. If the faxback doesn’t answer your question you can then submit an email to CustomerServiceCorrespondence@WEAtrust.com. When using email for communication, turnaround times may take up to 10 business days for a response.
